Seller’s Reply to Proposed A/S-Buyer’s Property (11/01)
Used with Sale and Settlement of Other Property Contingency Addenda (Form SSP) to reply to offers received by Buyer for Buyer’s property.
SOPSettlement of Other Property Contingency (12/01)
Where Buyer has an Agreement to sell his current property but hasn’t yet closed, this makes a transaction contingent on settlement of Buyer’s property.

Use when the leases for a tenant-occupied property are unavailable at the time the Agreement of Sale is drawn up. The offer becomes contingent on the buyer reviewing the written leases and memoranda of all oral leases for the tenant-occupied property.
ZA Zoning Approval Contingency Addendum to Agreement of Sale (9/05)

FYPFor Your Protection: Get a Home Inspection
Mandatory form to be signed by buyers seeking financing through an FHA lender. This is a change to the FHA language currently included in the Agreement of Sale. Brokers may provide the form if FHA financing is anticipated during the drafting of the Agreement of Sale; otherwise the lender will provide the form as part of the process of creating the loan file.
RELAgreement of Sale Release (12/01)
Releases Buyer and Seller from the obligations of the Agreement of Sale.
BDABroker’s Disclosure Addendum to A/S (6/00)

XLR Exclusive Listing for Lease of Real Estate (11/00)
Listing contract for rental property. 
XLS Listing Contract–Residential (11/03)
NBA Non-Exclusive Buyer Agency (2/04)
An alternative to the exclusive buyer agency contract in the Business Relationship form, this form can be used as a non-exclusive buyer agency contract, or as a memorandum of the terms of an oral agreement. 
BFA Broker’s Fee Agreement (2/02)
Use when working as a Buyer Agent or a Transaction Licensee in a For Sale By Owner transaction. 
CBC Cooperating Broker Compensation Agreement (6/04)
Provides for the Buyer Agent, Transaction Licensee, or the Subagent for Seller to be paid by the Listing Broker. Allows for the acceptance of the Listing Broker’s offer of compensation or modification of the offer. Form is not to be used as an addendum to the agreement of sale.

When used in conjunction with the BR or the XLS, fulfills legal requirements that certain items must be disclosed whenever a licensee charges a fee to a consumer.

Vacant Land Addendum to Listing Contract (11/03) 
Use when listing a piece of vacant land. Form has been revised to reflect the changes in the newly revised Residential Listing Contract (Form XLS).
XLR-A Property Description Addendum to XLR (7/97)

SA Single Agency Addendum to BR & XLS (11/99)

SCN Consumer Notice (Spanish Version) (4/02)
Full text of Consumer Notice translated into Spanish. 
CNT Consumer Notice for Tenants (4/02)
Used instead of the Consumer Notice if a licensee is showing a rental property to prospective tenants, if the licensee is 1. the actual owner or landlord of the property; or 2. the direct employee of the owner or landlord of the property; or 3. leasing the property pursuant to a property management agreement or an exclusive leasing agreement with the owner/landlord. If a rental application is required by the licensee, the form may be provided at the time the rental application is taken instead of at the first substantive discussion (see Rental Application, Form RA) 
DFI Disclosure of Financial Interests (11/99)
Fulfills the legal requirement that a licensee must disclose any financial interest that the licensee or an affiliated licensee has in any service provided to the consumer by any other person. 
LPA Residential Lead-Based Paint Hazards Disclosure and Inspection Contingency Addendum (9/05)
Mandatory for use with all properties built prior to 1978. New form to be used with the new Agreement of Sale (Form A/S-R) (9/05).
Oral Consumer Notice The Oral Consumer Notice must be read word-for-word to a consumer when the first substantive conversation is over the phone. The broker/agent must follow up with the regular Consumer Notice (Form CN) at the first face-to-face meeting.
SPD Seller’s Property Disclosure Statement (6/04)

Use when listing commercial real estate for sale and/or lease.

Used to provide good faith estimate of Buyer’s costs at closing. Includes Deposit Money Notice to Prospective Buyer (Form DMN). Revised for use with the new Agreement of Sale (Form A/S-R)(9/05).
BFI Buyer’s Financial Information (6/01)
Used by Buyer’s Agent to get overview of Buyer’s financial situation.
DMN Deposit Money Notice to Prospective Buyer (5/01)
Required to be used when Buyer’s Broker is holding deposit monies prior to delivery to Listing Broker. 
DRS Dispute Resolution System (DRS) Brochure (9/98)
Introduction, Guide to Initiating and the Rules and Procedures of Mediation. The Rules and Procedures that must be given to Buyer and Seller are also contained in the Agreement of Sale. Use this brochure with Agreement to Mediate (Form ATM) or as separate informational piece about the Dispute Resolution System. 
ESC Escrow Agreement (11/99)
For use when money must be placed in escrow in order to proceed with settlement because inspections, repairs, or other requirements will be finalized at a date later than settlement.

PAR Residential Lease (LR) and the Agreement of Sale (A/S-2K) fulfill the rest of the Act’s requirements.
